Microfilm/Scanning Order Form (PDF, opens in a new tab) — Fill out the Microfilm/Scanning Order Form and email, fax, or mail a copy to the Delaware Public Archives. Payment may be mailed, or credit card information may be provided.

School Records Release Form

School Release Form (PDF, opens in a new tab) — For Henry C. Conrad, Wilmington High, and William C. Jason. This form authorizes the disclosure of education records. When submitted, this authorization becomes part of the student permanent record in accordance with the Family Educational Rights and Privacy Act of 1974 and P.L. 94-142, Education for All Handicapped Children Act.

If a school is still in operation, patrons must request educational records through the school. The Delaware Public Archives only services record requests for high schools that are permanently closed. Currently, we are able to assist with record requests for:
