About this guide:

This guide – part list, part index cards – indexes the Department of Health and Social Services, Vital Statistics, Death Records collection. The records were maintained by the Bureau of Vital Statistics and its predecessors from 1700 to 1999, and the indices were created by DPA.

Listed below is this index, separated into alphabetical PDF files. Each file contains the index card scans and the list, for each letter beginning the last name of the subjects. These PDFs can be quite large, but were OCR’d for easy searching.
